Skin Diseases
Eczema, Dermatitis and Psoriasis
Chronic inflammatory skin conditions. They cannot be cured, but they can be controlled, and the difference between being controlled and not is huge for quality of life.
Atopic dermatitis and eczema
Atopic dermatitis is characterised by dry skin, intense itching and inflammatory lesions that appear in flare ups. Itching is often the most disabling symptom, interfering with sleep and concentration.
There is a defect in the skin barrier that makes the skin more permeable to irritants and more prone to losing water. That is why daily moisturising is not an add on: it is central to treatment, even between flare ups.
Psoriasis
Psoriasis shows up as reddish plaques with whitish scaling, typically on the elbows, knees, scalp and lower back.
It is a systemic disease with skin manifestations and may be associated with joint involvement. That screening is part of the assessment, because psoriatic arthritis benefits from early diagnosis.
Treatment
The strategy rests on three pillars: control inflammation during a flare, maintain the skin barrier between flares, and identify the factors that trigger them.
- Topical anti inflammatory treatment, adjusted to the area and severity
- Emollients for daily, continued use
- Systemic treatment in moderate to severe cases
- Identification of irritants, allergens and aggravating factors
Frequently asked questions
Does atopic dermatitis go away with age?
In many children it improves substantially as they grow. In other cases it persists into adulthood or comes back. A tendency to dry, sensitive skin usually remains even when flare ups become less frequent.
Are topical corticosteroids dangerous?
Used correctly, with the right strength for the area and for the indicated time, they are safe and effective. Problems arise with prolonged, unsupervised use. Fear of using them often leads to longer flare ups than necessary.
Does stress make it worse?
It is a recognised trigger, both in atopic dermatitis and psoriasis. It is not the cause of the disease, but it influences how often and how intensely flare ups occur.
